BNA Commercial Lease Credit Application: click on the icon above, fill in all of the highlighted cells while online, and then print the document. Sign the form at the bottom-left, then fax to BNA’s office in Aurora, Ontario 905-726-9751 or 1-866-374-9438. You will be able to Save the filled-in form if you have a copy of Adobe Professional, only, on your computer. Otherwise, please retain your faxed copy. You must also fax a copy of a void cheque for the account to be used for lease payment deduction
BNA Lease Contract: click on the icon above, fill in all of the highlighted cells while online, and then print the document. There are THREE (3) signatures required on every Lease Contract. On the First Page a signature is required a) next to “Guarantor Signature”, five lines from the bottom-right of the page, and b) next to “By” at the bottom-middle of the form. Another signature is required, along with Title, on the Third Page, which signifies acceptance of BNA’s Pre-Authorized Payment Plan (PAPP) on the line underwritten with “Authorized Cheque Signing Officer (Title)”. Note: PAPP is mandatory. Once this is completed, please fax to BNA’s office in Aurora, Ontario 905-726-9751 or 1-866-374-9438. You will be able to Save the filled-in form if you have a copy of Adobe Professional, only, on your computer. Otherwise, please retain your faxed copy.
